What is Botox and how does it work?


Botox® is the trade name for a prescription medicine (Botulinum Toxin A) which is used in cosmetic medicine to create a fresher and more youthful appearance by temporarily relaxing facial muscles.

When you frown or concentrate, the muscles between your brows contract, causing your skin to furrow and fold. When you squint, the muscles around your eyes contract and cause crow's feet. Botox ® works beneath the skin's surface to target the underlying muscle responsible for causing the lines to form. It relaxes these muscles to temporarily improve, reduce or eliminate the look of lines and wrinkles, resulting in a refreshed, rested and more youthful look.

It is primarily used to treat:

  • Worry Lines: The horizontal lines on the forehead

  • Frown Lines: The vertical lines between the eyes

  • Crows Feet: The lines at the lateral aspect of the eyes

  • Bunny Lines: The lines on the lateral aspect of the nose

  • Downturned Smile

  • Neck laxity

  • Jaw slimming

  • Grinding/Bruxism

  • Underarm sweating




Are there different types of Botox?


No there is only one Botox. This is a brand name for a particular type of Botulinum Toxin A. Other brands of Botulinum Toxin that you may have heard of include Dysport - Azzalure and Xeomin -Bocouture. Each is very well tested with long clinical history. At ORA we primarily use Allergan Botox.




Who may not be suitable for Botulinum Toxin A ?


  • Pregnant or trying to get pregnant
  • Breast Feeding
  • Neuromuscular disorder (EG: Myasthenia Gravis/Eaton Lambert Syndrome/Multiple Sclerosis/Parkinsons/Epilepsy)
  • Infection including acne at the site of injection
  • Presence of cold sores anywhere on the face
  • History of Bells Palsy
  • Allergy to Botulinum Toxin or Albumin (egg)

What should I not do before my Botulinum Toxin A appointment?


It is important that you avoid the following in advance of your scheduled appointment:

  • No alcohol (24 hours). Alcohol in the 24 hours pre appointment increases the risk of bleeding which in turn increases the risk of bruising.
  • No sunbeds or extremem heat (2 weeks).
  • No microneedlign or active facials (2 weeks).
  • If possible avoid wearing make-up to your appointment.




What can I not do after my Botulinum Toxin A appointment?


  • No lying down (4 hours)
  • No make-up or touching the treated area (24hours)
  • No alcohol (24 hours)
  • No vigorous exercise (48 hours)
  • No Sunbeds (2 weeks)
  • Avoid extreme heat/cold (including sauna/steam rooms etc (2 weeks)
  • No microneedling/skin peels/active facials (2 weeks)




Does getting Botulinum Toxin A hurt?


At ORA we use very tiny needles to administer Botulinum Toxin A precisely and as painlessly as possible. You will feel small pinpricks during the procedure.

We also use different needles for each area to ensure the needle does not blunt. A blunt needle can increase pain.

Pain perception can also be heightened around the time of your menstruation so it is a good idea to bear this in mind when booking appointments.




When will I start to see results?


Initial results are typically seen after 1 week but final results are not seen until 2 weeks post treatment. This is also the time when we offer all patients a review to evaluate the results and carry out any further complimentary treatment.




How long do the results last?


The effects of Botulinum Toxin A are temporary and how long it will last depends to some extent on individual client factors. For most people the results last between 3-4 months however runners or people who train a lot tend to metabolise Botulinum Toxin A at a faster rate. Pharmacologically Botulinum Toxin A starts to be broken down by the body seven weeks post treatment. At this stage you may start to see a gradual increase in movement. The good news is that with repeated doses the lines and wrinkles often appear less severe because the muscles being treated are being trained to relax. Botulinum Toxin A treatment is best taken as a course with repeat treatment every 3-4 months. This ensures results are always natural.




How often should I have Botulinum Toxin A ?


Results usually last 3-4 months but individual variation is possible. Some people find it lasts longer and others find it wears off a little Quicker. The duration is dependent on dose, how much you use the muscles we are trying to relax and on your metabolism. This will be discussed in your consutation at ORA.




If I start having Botulinum Toxin A will I have to keep having it done?


Of course but the vast majority of patients love the results so much that they couldn’t imagine going back to their pre-Botulinum Toxin A days!




Can Botulinum Toxin A be reversed?


No, Botulinum Toxin A cannot be reversed but it does wear off. It typically lasts 3-4 months but it can last up to 6 months for some patients.




What are the risks of having Botulinum Toxin A ?


Botulinum Toxin A is a medical procedure and should only be administered by appropriately trained medical professionals. Botulinum Toxin A is an extremely safe procedure with a safety record spanning 20 years. Nonetheless, it is still important to remember that because it is a medical procedure there are risks and potential side effects.

  • Any time that the skin is pierced with a needle – you risk infection, swelling and bruising, even if it’s only a minor risk. It is advisable not to wear makeup or touch the treated area for 24 hours after treatment.
  • Headache. Up to 10% of patients can experience a headache. Some patients have described it as a feeling of heaviness in the forehead region in the first week of treatment. This typically resolves within the first 14 days.
  • Eyebrow or eyelid ptosis. These are the most undesirable side effects. This is where either the eyelid or eyebrow visibly drops. True ptoses are reported to occur in approximately 3% of cases and can be due to regional anatomical variation, injector technique, poor compliance with post-operative advice or a combination of factors. If you have had a ptosis previously then you are more likely to have it occur again. Most ptoses resolve within 6 weeks.
  • Double vision. This is rare but it can occur.




Is there any downtime after Botulinum Toxin A treatment?


You may have small bumps at the injection sites. These will resolve within 30 minutes.

There are a couple of things you should not do after treatment. For example, you should avoid strenuous exercise for 24 hours, avoid make-up for 4 hours, avoid other skin treatments and excessive heat such as sun beds and stream rooms for 2 weeks. Otherwise, you can return to normal immediately after treatment.

Many clients attend at lunchtime and return to work immediately post-treatment.




Will I look frozen?


Our philosophy at ORA is to achieve natural results, always. For us being frozen is never natural or beautiful so we will always educate our clients about the benefits of regular, layered treatments targeting all the signs of aging as opposed to just focusing on wrinkle relaxers.

When we administer wrinkle relaxer injections we temporarily relax the treated muscles to allow any wrinkles caused by the constant movement of these muscles to gradually soften after treatment. The overlying skin remains smooth and unwrinkled while the untreated facial muscles continue to work normally so facial expression is unaffected by the treatment.

Patients look the same as they always have done, only more youthful and rested. We also offer all patients a review where results can be tweaked if needed. This is the most reliable and effective way of achieving subtle beautiful results. We can always add more product but if overtreated it cannot be removed!




Is it true that Botulinum Toxin A only works when you are a certain age?


Whilst it is true that the more elastic the skin, the better the treatment will work – Botulinum Toxin A can usually be utilised to smooth wrinkles from around 20 to 60 years of age. It works best in patients who have lines caused by muscle pull (and preferably before the lines have become too deep), as opposed to wrinkles caused largely by excess loose skin and/ or sun damage. An individual consultation is the best way of ascertaining whether the treatment is suitable and beneficial for you as an individual patient.




Does Botulinum Toxin A prevent wrinkles?


Yes, Botulinum Toxin A helps prevent the formation of new dynamic lines and stops dynamic lines that have already been formed from deepening. The rationale for starting Botulinum Toxin A early is the prevention of the permanent development of deep wrinkles or lines.

The average woman begins to show signs of facial aging around the age of 25. The first signs of aging in women are new lines around the eyes (crow’s feet) and around the mouth, as well the “elevens” – the frown lines between the eyebrows. Women tend to show the first signs of aging younger than men because a women’s facial skin is thinner. Men have thicker skin and an increased blood supply to the skin of the face. For this reason men tend to start using Botulinum Toxin A later in life; around their mid-thirties.

As Botulinum Toxin A can have a preventative effect, it’s best not to wait until your wrinkles or lines are deeply etched on the face and then expect an overnight miracle!

Can I have my treatment and consultation on the same day?


Of course, this is entirely up to you. Many patients like to have treatment and consultation on the same day!




I had Botulinum Toxin A recently, but I still have lines on my face?


It is very important to remember that every face is different. How your muscles react to treatment, how deep your lines were pre -treatment and whether or not the lines were caused by dynamic movement all influence the outcome. At ORA we like to build long-term relationships with our clients where, with time, we know exactly what your unique muscles need. We can also assist and guide you in formulating an appropriate skin treatment plan to maximise results.

The most important thing to remember is that static lines are not improved with Botulinum Toxin A . This is because Botulinum Toxin A works on the muscle so only those lines created by muscle movement can be treated with Botulinum Toxin A. Static lines can be created for by sun-damage or excess skin.

Another important thing to remember is that where dynamic lines were particularly deep it is possible to see an imprint of these lines post-treatment. This is because the skin in this area is photo-damaged. You will need to combine regular Botulinum Toxin A treatments every 3-4 months with an excellent skin care routine to eliminate these lines. What is your policy in regards to top-ups?


All ORA clients are offered a review appointment within 3 weeks of treatment and we are always more than happy to administer more Botulinum Toxin A if requred.

Unfortunately no top-ups will be performed more than 3 weeks (21 days) post treatment. We cannot ‘top-up’ after this point as it would increase your risk of developing an immunity to Botulinum Toxin A years in the future and for this reason it is not a risk worth taking!

Is there anything else I should do alongside Botulinum Toxin A to help me to look my best?

  • S M O K I N G: Smoking is known to cause premature wrinkles. How? The nicotine in cigarettes causes the narrowing of blood vessels in the outermost layers of your skin. This impairs blood flow to your skin. With less blood flow, your skin doesn’t get as much oxygen and nutrients such as vitamin A. Also, many of the over 4000 chemicals in tobacco smoke also damage collagen and elastin which are the fibers that give your skin its strength and elasticity. As a result, skin begins to sag and wrinkle prematurely. And last but not least, the repeated exposure to heat from burning cigarettes and the facial expressions you make when smoking – such as pursing your lips when inhaling and squinting your eyes to keep smoke out – may contribute to wrinkles.
  • D I E T: Eating a diet rich in fruit & vegetables is important for skin health. Fresh fruits & vegetables are a great source of the vitamins your skin needs such as vitamin A and Vitamin C. Some studies also show that Flaxseed oil which is a rich source of alpha-linolenic - acid can help in the prevention of wrinkles.
  • S K I N C A R E: Invest in a good skincare routine. We work with OBAGI and AlumierMD to create tailored skin treatments that include in-clinic and home-care.
  • S L E E P: Sleep is vital for our bodies to repair. Without sleep you cortisol level increases which in turn decreases cell turnover and speeds up the ageing process.
